Our Take
Sepsis diagnostics is a market built on an old, slow method, the blood culture, and on a simple clinical fact: every hour of delay in effective antibiotics costs lives. The market grows from $1,290 million in 2026 to $2,100 million in 2031, at 10.2% a year. Blood culture remains the largest technology, but faster answers take the growth: direct-from-blood tests, rapid susceptibility testing and host-response tests grow from $296.6 million to $714.0 million, more than half of the $810 million of new spending.

Market Size 2026: USD $1.29B Market Size 2031: USD $2.1B CAGR: 10.2%
Situation Analysis: what is really happening
- Payment now follows outcomes. SEP-1 counts in Medicare's Hospital Value-Based Purchasing payments from October 2025, and the FY2027 hospital rule adds a sepsis readmission measure with penalties from FY2030.
- The guidelines got stricter. The 2026 Surviving Sepsis Campaign guidelines suggest procalcitonin for stopping antibiotics, not for starting them, and found insufficient evidence for new host-response tests.
- Blood culture has a new owner. Waters completed its combination with BD's diagnostics business, taking over BACTEC and Phoenix, and won FDA clearance for the BACTEC FXI system.
- Rapid susceptibility is the growth engine. Rapid AST grows 17.6% a year, from $135.4 million to $304.5 million; Bruker bought the former Accelerate Diagnostics assets and Q-linea expanded its ASTar menu.
- Direct-from-blood testing grows fastest. It rises 22.4% a year, from $103.2 million to $283.5 million, despite the wind-down of T2 Biosystems; Karius is now orderable through Mayo Clinic Laboratories.
- AI reaches the bedside. Bayesian Health's sepsis early-warning system was cleared by the FDA and won a Medicare new-technology add-on payment.

Company Analysis: who is positioned, who is exposed
- bioMérieux, Waters (formerly BD Diagnostic Solutions) and Bruker: Own the microbiology workflow, from blood culture to identification and susceptibility testing.
- Roche, Abbott, Siemens Healthineers, Beckman Coulter, Radiometer and Thermo Fisher: Core laboratory and acute-care suppliers of procalcitonin, lactate and other sepsis biomarkers.
- Qiagen, Cepheid and DiaSorin: Molecular blood culture identification panels, with Qiagen completing its US panel set in 2026.
- Inflammatix, Cytovale, Immunexpress and MeMed: Host-response tests that must still win over guideline writers.
- Karius, Q-linea, Gradientech, Scanogen, Bayesian Health and Prenosis: Innovators in sequencing, rapid susceptibility and AI.
- Mindray, Snibe, Autobio, Zhuhai DL Biotech, Getein and Wondfo: Chinese suppliers of blood culture, susceptibility and biomarker tests, growing in their home market.
In all, 38 company profiles, each one to two pages with a fact box covering website, headquarters, ownership, revenue, employees and key products.
